What You Need to Know About Your First Visit

DSC_0069.jpgThank you for choosing Midwest Eye Institute. We look forward to the opportunity to participate in your medical care. During your first visit, our goal is to get to know you better, to become familiar with your health history, and to establish a treatment plan that will best meet your eyes need.

Things to Know Prior to Your Visit:

  • It will save you and your physician time if you complete your new patient paperwork in advance. You can do so by clicking here to complete our online registration, or click here to print out a new patient packet.
  • You are responsible for obtaining any required authorization from your insurance carrier.
  • Be sure to verify with your Midwest Eye physician’s office when you schedule your first visit that they are positioned to accept any third-party insurance coverage you plan to use for your visit.
  • Though we respect our patients’ time, and we do not like to keep anyone waiting unnecessarily, it is not possible to know in advance exactly how long each appointment will take. There is always the possibility of unforeseen emergencies arising for your physician on the day you are scheduled to be seen.
  • Allow at least an hour to two hours for your first appointment. Depending on what you are being seen for, your appointment could take as long as four hours.
  • If you are diabetic, please take regular medications and plan ahead.
  • Chances are strong your eyes will be dilated during your visit, so we suggest you make arrangements to have someone pick you up, or bring someone with you who can drive you home.

Things to Bring for Your Visit:

  • A valid picture ID
  • Health insurance information/cards
  • Any medical records regarding the problem we are seeing you for.
  • Medication list (if any)
  • Co-payments (if any). Check with your Midwest Eye physician’s office directly to find out what forms of payment they can accept.

If you are unable to keep your appointment at any time, please notify the office as soon as possible. A minimum of 24 hours advance notice is ideal.
