You will of course be required to provide any health insurance information/cards upon your arrival. And a valid picture ID is also important on your first visit. We ask for a valid ID in order to help protect you against fraudulent use of your health insurance card/account. We also request that you bring with you any medical records regarding the problem we are seeing you for.
Most of our Midwest Eye providers participate in the Medicare program; and many also participate in a variety of other widely accepted insurance programs. Please be sure to verify with your Midwest Eye physician’s office when you schedule your first appointment that they are positioned to accept any third party insurance coverage you plan to use for your visit with us.
It will save you and your physician time if your complete your new patient paperwork in advance of your visit. Most of the necessary forms are available from this website. Please consult with our staff when you schedule your appointment about which documents you should complete in advance and bring with you to your clinic visit. It would also be helpful if you could bring a listing of all your current medications to your first visit.
Though we respect our patients’ time, and do not like to keep anyone waiting unnecessarily, it is not possible to know in advance exactly how long each appointment will take. And of course there is also the possibility of unforeseen emergencies arising for your physician on the day you are scheduled to be seen. Therefore we recommend you allow at least an hour and a half to two hours for your first appointment. Depending on what you are being seen for, your first appointment could take as long as four hours! Please ask about this when you speak to our staff prior to your visit. Diabetic patients, or patients taking regular medications, must plan ahead and adjust their needs accordingly.
Prior to your visit you are responsible for obtaining any required authorization from your insurance carrier. You will also be expected to make any office visit co-payments at the time of your visit; and you may be asked to make any other co-payments for the services provided during your visit. Please be prepared with a valid form of payment, e.g. cash, personal check, debit/credit card. These responsibilities apply not only to your first visit, but also to any subsequent visit with one of our practices/physicians. If you need assistance filing a medical claim your physician will have staff that can help.
Chances are strong your eyes will be dilated during your clinic visit, so we suggest you make arrangements to have someone pick you up; or bring someone with you who can drive you home.
If you are unable to keep an appointment – at any time – we ask that please notify as soon as possible. A minimum of 24 hours advance notice is ideal.
